Connected Membership: Starting at $235/month

We offer a convenient membership plan to simplify pricing and assure your clinician is readily available to you. Connected Membership helps you connect with your optimal health and wellness, and lets you stay connected with your clinician.

• Personalized management of a wide variety of acute and chronic health conditions

• 60 minute comprehensive new patient intake visit

• Monthly 30 minute follow-up visit with clinician

• Custom Health and Wellness Plan sent to you after each visit

• Lab ordering and review

• Supplement and medication recommendation/prescribing

• Regular communication with your clinician through our secure patient portal or, in cases of needing to schedule an urgent care visit (see below), by text.

Add-on services available for members only:

15 minute virtual urgent care visit (for single concerns such as UTI, sinus infection, skin rash, flare of chronic back pain, etc.): $45

Points to note about pricing:

• A nonrefundable deposit of 50% of visit fee is required at the time of booking New Patient Package appointments as well as Connected Membership urgent care visits and additional 30 or 60 minute visits. Deposit will be applied to future visit in the case of a patient needing to reschedule.

• *Cost of labs not included in New Patient Package or Connected Membership. Most patients use insurance to cover cost of labs. It is the responsibility of the patient to communicate and coordinate with their insurance plan regarding labs coverage. Specialty labs such as GI-MAP, some nutritional and food allergy panels, and adrenal function testing typically not covered by insurance. These labs are available for an additional fee and coordinated through our affiliate specialty laboratories.

• Cost of supplements and prescription medications not included in New Patient Package or Connected Membership. Whenever possible we help our patients use insurance or secure coupons to pay for medications and and supplements. Many supplements can be purchased over the counter from one of our recommended brands, or with discounted pricing for Connected Wellness Center patients through our affiliated supplement suppliers or compounding pharmacy. Many supplements and prescriptions medications can be delivered directly to your home from our specialty pharmacy. We are glad to send to retail pharmacies any prescriptions available in the the formulation and dosing we advise.

• Membership fees are self pay only. When requested we will provide a Superbill including diagnostic codes for patients to use to request insurance or HSA/FSA reimbursement. It is the responsibility of the patient to communicate with their insurance and/or savings plan provider to determine whether their plans reimburse or pay for our services.
